Southern Administrators and Benefit Consultants, Inc. (SABC) and its employees, believe in protecting the confidentiality and security of information we collect about you. This notice refers to SABC and its employees, by using the term "us", "we", or "our." This notice describes our Privacy Policy regarding nonpublic personal and protected health information ("information") that we may collect and disclose. This Notice of Privacy Practices has been drafted to be consistent with what is known as the "HIPAA Privacy Rule," and any of the terms not defined in this notice should be consistent with what is known as the "HIPAA Privacy Rule," and any of the terms not defined in this notice should have the same meaning as they have in the HIPAA Privacy Rule. Our Privacy Policy will apply from the time our relationship begins, during the course of our customer relationship, as well as after the relationship ends.
Nonpublic personal information, as we used in this notice, means information that identifies an individual personally and is not otherwise available to the public. It includes personal financial information such as premium amounts, flexible spending elections, employment, dependent and claim information. It also includes personal health information such as individual medical records and information about illness or injury.
How We Protect Information. Except as explained below, we restrict access to nonpublic personal information about you to our employees who need to know that information to provide products or services to you. We maintain physical, electronic, and procedural safeguards that comply with federal and state regulations to guard your nonpublic personal information.
Information We Collect and Maintain. We collect nonpublic personal financial information to support our normal business operations. We gather personal financial and medical information such as name, address, social security number, salary, product premiums, medical drugs and procedure information from sources as needed to comply with Section 125 Cafeteria Plan requirements. We may collect such information directly from your employer and from you.
Information We Share. We may share any nonpublic personal financial information, as described above, with non-affiliated third parties, (Enrolling Cafeteria Plan Agents as approved by the Company's Plan Administrator). In addition, we may share any nonpublic personal financial information, as described above, with other financial service providers, such as approved enrolling agents and companies that perform services on our behalf such as data programming and storage companies. Where we make these disclosures, we require those non-affiliated third parties to protect the confidentiality of such information and to use it solely for the purposes for which we disclosed the information, except as permitted by law. Otherwise, we do not disclose your nonpublic personal financial information to non-affiliated third parties.
Health Information. We may disclose personal health information only with proper written authorization or as otherwise permitted or required by law.
Our Responsibilities. We are required by law to maintain the privacy of your nonpublic and protected health information. We are obligated to provide you a copy of this notice of our legal duties and of our privacy practices with respect to protected health information, and must abide by the terms of this notice.
